Lifetime Membership Nomination Form

The club may award up to one member per year a lifetime membership. This award is in recognition of significant contributions to the club. This award does not have to be awarded every year.

Qualifications

To be considered for the club's lifetime membership award a person must have been a member for at least 5 years, and have made significant contributions to the club.

Nomination Process

Any member of the club may propose any other member for the lifetime membership award. Nominations should be sent to the chair of the Lifetime Award Committee and should include the reasons in support of the nomination.
